Nanoliposomal irinotecan is a topoisomerase 1 inhibitor formulated as a nanoparticle liposome encapsulation of the active ingredient irinotecan. This formulation is designed to prolong circulation time and enhance delivery to tumor tissue compared to conventional (non-liposomal) irinotecan. The drug acts by inhibiting topoisomerase 1—leading to DNA damage and cell death in rapidly dividing cancer cells. Nanoliposomal encapsulation aims to maximize anti-tumor efficacy while minimizing systemic toxicity. It is primarily indicated for the treatment of metastatic pancreatic ductal adenocarcinoma (mPDAC), both as first-line therapy in combination with oxaliplatin/fluorouracil/leucovorin (NALIRIFOX regimen) and after progression on gemcitabine-based therapy in combination with fluorouracil/leucovorin[1][2][3][4][5][7].
