Nanoliposomal irinotecan + carboplatin is an investigational combination therapy consisting of nanoliposomal irinotecan (a liposome-encapsulated formulation of the topoisomerase I inhibitor irinotecan) and the platinum-based chemotherapeutic agent carboplatin. Nanoliposomal irinotecan delivers its active metabolite SN-38 more efficiently to tumor cells, enhancing DNA damage by inhibiting topoisomerase I, which leads to double-strand DNA breaks and apoptosis. Carboplatin forms DNA crosslinks that inhibit DNA replication and transcription, resulting in cell death. This combination is being studied as a first-line treatment for patients with locally advanced or metastatic gastroenteropancreatic neuroendocrine carcinomas (GEP-NECs)[1][2]. The primary aim is to define dose-limiting toxicity and establish a recommended phase II dose.
