Nanrilkefusp alfa is a recombinant human fusion protein that acts as an interleukin (IL)-15 receptor βγ superagonist. It consists of the cytokine IL-15 fused to the high-affinity sushi+ domain of the IL‑15 receptor alpha (IL‑15Rα). This design selectively stimulates cytotoxic lymphocytes, specifically natural killer (NK) cells and CD8+ T cells, while avoiding expansion of regulatory T cells and minimizing activation-induced cell death. Nanrilkefusp alfa promotes both innate and adaptive anti-tumor immune responses by increasing NK cell and memory CD8+ T cell levels in peripheral blood and tumors. It has been investigated as a subcutaneously administered immunotherapy for advanced solid tumors, both as monotherapy and in combination with checkpoint inhibitors such as pembrolizumab or cetuximab[1][2][3][4][5][6][7]. The drug was developed to improve efficacy by limiting T cell exhaustion through optimized half-life extension[3].
