**Nanrilkefusp alfa + cetuximab** is a combination therapy comprising nanrilkefusp alfa (previously SOT101), a recombinant fusion protein and interleukin-15 (IL-15) receptor βγ superagonist, and cetuximab, an anti-EGFR monoclonal antibody. Nanrilkefusp alfa is designed to stimulate and expand cytotoxic T cells (especially CD8+ T cells) and natural killer (NK) cells, promoting an innate and adaptive anti-tumor immune response. Cetuximab functions as a chimeric monoclonal antibody targeting the epidermal growth factor receptor (EGFR) and mediates antibody-dependent cellular cytotoxicity. The rationale for the combination is that IL-15 agonism by nanrilkefusp alfa can enhance both direct antitumor T cell activity and the cytotoxic effects mediated by cetuximab-activated NK cells, potentially increasing antibody-dependent cellular cytotoxicity. This combination was explored in patients with RAS wild-type metastatic/advanced colorectal cancer, specifically those refractory/intolerant to standard chemotherapy. However, clinical development in this setting has been discontinued due to insufficient efficacy observed in interim analyses[1][3][4][6].
