Overview
**Napabucasin + gemcitabine + nab-paclitaxel** is a combination chemotherapy regimen investigated for the first-line treatment of metastatic pancreatic adenocarcinoma. **Napabucasin** is an investigational oral small molecule drug that acts as a reactive oxygen species (ROS) generator and is bioactivated by NAD(P)H:quinone oxidoreductase 1 (NQO1), targeting cancer stem cell pathways and inhibiting multiple oncogenic signaling pathways, including signal transducer and activator of transcription 3 (STAT3)[1][3]. **Gemcitabine** is a nucleoside analog (antimetabolite) that inhibits DNA synthesis, causing cytotoxicity in rapidly dividing cells[3][4][8]. **Nab-paclitaxel** is a nanoparticle albumin-bound formulation of paclitaxel, a microtubule inhibitor disrupting cell division, designed to improve delivery and reduce solvent-related toxicity[4][8]. The regimen was studied in phase 3 trials (CanStem111P) but did not show improved survival compared to gemcitabine plus nab-paclitaxel alone[1][3].
