Overview

Naropeine (a brand name for ropivacaine) and levobupivacaine are long-acting amino-amide local anesthetics. Ropivacaine is the pure S-(-)-enantiomer of 1-propyl-2',6'-pipecoloxylidide, while levobupivacaine is the S-(-)-enantiomer of bupivacaine. Both drugs exert their effect by reversibly blocking voltage-gated sodium channels in nerve fibers, preventing the influx of sodium ions and thereby inhibiting the initiation and conduction of nerve impulses. This results in a reversible loss of sensation and, at higher concentrations, motor blockade. These agents are characterized by a lower potential for central nervous system and cardiovascular toxicity compared to racemic bupivacaine. In the AGORA trial sponsored by Elsan, these agents are utilized for loco-regional anesthesia (specifically peripheral nerve blocks) in patients undergoing rotator cuff repair surgery to evaluate blood pressure stability and clinical outcomes compared to combined general and loco-regional anesthesia.
