Overview

NB-4746 is a first-in-class, oral, brain-penetrant small molecule developed by Nura Bio as a neuroprotective therapy for neurological diseases. It acts as an inhibitor of SARM1 (sterile alpha and TIR motif containing 1), a neuronally enriched NAD hydrolase that serves as an axon-intrinsic metabolic sensor and central driver of axon degeneration. By inhibiting SARM1, NB-4746 aims to halt or prevent axonal degeneration—an early hallmark in several neurological disorders—thereby providing structural and functional neuroprotection. Preclinical studies have shown that NB-4746 confers significant protection in models of amyotrophic lateral sclerosis (ALS), multiple sclerosis (MS), traumatic brain injury (TBI), and chemotherapy-induced peripheral neuropathy (CIPN). The drug has demonstrated the ability to reduce or prevent increases in neurofilament light chain (NfL), a biomarker associated with nerve cell damage. As of June 2025, it has completed Phase I clinical trials evaluating safety, tolerability, pharmacokinetics, and brain penetration in healthy volunteers; further patient trials are planned[1][2][4][5][6][7][8].
