NC18 is a novel HER2-targeting antibody-drug conjugate (ADC) developed by Novacyte Therapeutics. It consists of a humanized monoclonal antibody directed against HER2, conjugated via a PHF (poly-1-hydroxymethylethylene hydroxymethyl-formal) polymer linker to AF-HEA, a tubulin-inhibiting payload that is an analog of Auristatin F (MMAF). NC18 is designed to treat HER2-expressing advanced solid tumors, including breast cancer, gastric cancer, and gastroesophageal junction adenocarcinoma. Preclinical data indicates that NC18 maintains anti-tumor activity in tumor models resistant to trastuzumab deruxtecan (Enhertu), suggesting it may provide a therapeutic option for patients who have developed resistance to existing HER2-targeted ADCs. The drug is currently in Phase 1 clinical development.
