Overview

NC181 is a novel humanized monoclonal antibody developed by NextCure that targets apolipoprotein E4 (ApoE4), a key genetic risk factor associated with Alzheimer's disease (AD). ApoE4 promotes amyloid-beta plaque formation, tau spreading, blood-brain barrier disruption, neurovascular leakage, and microglia-mediated inflammation leading to cognitive decline. NC181 binds specifically to amyloid-associated ApoE4 and has demonstrated in preclinical models the ability to clear amyloid plaques and prevent further amyloid deposition. It also normalizes neuroinflammation and restores neuroimmune homeostasis while improving vasodilation in amyloid-laden blood vessels to reduce vascular toxicity and leakage. Additionally, NC181 is being investigated for its potential therapeutic effect on cerebral amyloid angiopathy (CAA), a condition involving vascular deposition of amyloid-beta that causes blood-brain barrier dysfunction and cognitive impairment. The drug differentiates itself from other anti-amyloid therapies by targeting ApoE4 directly rather than only the plaques themselves[1][3][4][5].
