NCX 1022 is an investigational small molecule drug that acts as a nitric oxide (NO)-releasing derivative of hydrocortisone. It was developed to combine the potent anti-inflammatory effects of glucocorticoids with the additional benefits conferred by NO release. The drug is designed to have improved safety and tolerability compared to standard hydrocortisone, notably lacking skin blanching after repeated use. Mechanistically, NCX 1022 functions as a **glucocorticoid receptor agonist** and **releases NO** at the site of inflammation, which modulates early inflammatory events such as leukocyte recruitment and adhesion in skin tissue. Preclinical studies demonstrated that NCX 1022 is more effective than hydrocortisone in reducing granulocyte recruitment, tissue infiltration by inflammatory cells, and disruption of tissue architecture in models of dermatitis[1][3][5]. Its primary indication has been investigated for atopic dermatitis and other inflammatory skin conditions[1][4].
