NCX 1236 is a preclinical-stage, orally administered small molecule designed as a nitric oxide (NO)-donating derivative of gabapentin. It was developed by Nicox for the treatment of neuropathic pain, including painful diabetic neuropathy. The compound is engineered to release both gabapentin and NO after oral administration. Gabapentin acts on voltage-gated calcium channels to reduce neuronal excitability, while the NO component is intended to enhance analgesic efficacy by modulating pain pathways involved in neuropathic pain. Preclinical studies demonstrated that NCX 1236 provided superior anti-allodynic effects compared to equimolar doses of gabapentin in models of lesion-induced and diabetic neuropathic pain[2][4][5].
