NCX 1741 is a preclinical-stage, nitric oxide (NO)-donating small molecule derivative of the phosphodiesterase type-5 (PDE5) inhibitor avanafil, developed by Nicox. It is designed to lower intraocular pressure (IOP) in patients with glaucoma and ocular hypertension. NCX 1741 acts through a dual mechanism of action: it releases NO, which activates soluble guanylate cyclase (sGC) to increase cyclic guanosine monophosphate (cGMP) levels, and it inhibits PDE5 to prevent cGMP degradation. This dual action enhances cGMP-mediated relaxation of the trabecular meshwork, facilitating aqueous humor outflow and resulting in rapid and sustained reduction of IOP.
