NCX 434 is a novel nitric oxide (NO)-donating derivative of triamcinolone acetonide, developed for ophthalmic use. It is designed to combine the anti-inflammatory effects of glucocorticoids with the vasodilatory and oxygenation-enhancing properties of NO donation. Preclinical studies have shown that NCX 434 enhances optic nerve head (ONH) oxygen saturation, reduces oxidative stress markers, and attenuates inflammatory cytokines more effectively than standard triamcinolone acetonide. Unlike conventional steroids, it does not significantly increase intraocular pressure (IOP), addressing a key safety concern in steroid-based ocular therapies. The drug has demonstrated efficacy in preclinical models of ischemia/reperfusion injury and diabetic macular edema (DME), suggesting potential for treating retinal diseases characterized by hypoxia and inflammation[1][3][4][5][6].
