NCX 470 is a novel nitric oxide (NO)-donating bimatoprost eye drop developed for the reduction of intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension. It is a second-generation prostaglandin analog that incorporates both a prostamide component (bimatoprost) and an NO-donating moiety within a single molecule. Upon ocular administration and exposure to esterases in the eye, NCX 470 is cleaved into its active metabolites—bimatoprost and a NO donor. Bimatoprost acts as an agonist at the prostaglandin F receptor (FP receptor), increasing uveoscleral outflow of aqueous humor; meanwhile, nitric oxide activates soluble guanylate cyclase to enhance trabecular meshwork/conventional outflow facility via cGMP signaling. This dual mechanism provides greater IOP lowering compared to standard prostaglandin analogs alone[1][3][4][5][6][8]. The drug has demonstrated non-inferiority and numerical superiority over latanoprost in phase III clinical trials for IOP reduction.
