The combination of ndesitertinib (TAK-279) and erythromycin refers to a clinical drug-drug interaction (DDI) study. Ndesitertinib is a highly selective, oral, allosteric tyrosine kinase 2 (TYK2) inhibitor that binds to the pseudokinase (JH2) domain of the enzyme, thereby inhibiting the signaling of pro-inflammatory cytokines such as IL-12, IL-23, and Type I interferons. It is being developed for the treatment of immune-mediated inflammatory diseases, including psoriasis and psoriatic arthritis. Erythromycin, a macrolide antibiotic, is utilized in this context as a moderate inhibitor of the cytochrome P450 3A4 (CYP3A4) enzyme. The primary objective of co-administering these agents in Phase 1 trials is to evaluate the impact of CYP3A4 inhibition on the pharmacokinetics of ndesitertinib, helping to determine its metabolic profile and establish safe dosing guidelines for patients receiving concomitant medications that affect the CYP3A pathway.
