Overview

Nebivolol + ramipril is a combination of two antihypertensive agents: nebivolol, a highly selective third-generation β1-adrenergic receptor antagonist (beta-blocker) with additional endothelial nitric oxide (NO)-mediated vasodilating properties, and ramipril, an angiotensin-converting enzyme (ACE) inhibitor that reduces the synthesis of angiotensin II, decreasing vasoconstriction, sodium and water retention, and sympathetic activity. The combination is used, mainly in Europe, for the treatment of hypertension and as part of cardiovascular risk reduction in patients with conditions such as heart failure, diabetes, and renal disease. There is a pharmacologically synergistic and complementary effect between β-blockers and ACE inhibitors on the sympathetic nervous system and the renin–angiotensin–aldosterone system, both central to cardiovascular risk modulation and disease outcomes[1][2]. The combination is usually prescribed as an extemporaneous (not yet single pill) mixture, though movement toward fixed-dose, single-pill combinations is ongoing to improve adherence[1][2][3].
