Neem leaf glycoprotein (NLGP) is a 47 kDa glycoprotein isolated and purified from mature leaves of the Neem tree (Azadirachta indica). It is a plant-derived immunomodulator with demonstrated antitumor properties in various preclinical murine models of carcinoma, melanoma, and sarcoma by enhancing antitumor immunity. Mechanistically, NLGP binds primarily to Dectin-1 receptors on antigen-presenting dendritic cells, initiating CARD9-mediated intracellular signaling that activates NFκB pathways and drives type 1 immune responses. This results in activation of cytotoxic CD8+ T cells, suppression of regulatory T cells, maturation of dendritic cells, regulation of cytokine-chemokine balance, and modulation of macrophage phenotype. NLGP can re-educate exhausted CD8+ T cells, decrease cancer stem cell properties, and sensitize tumors to chemotherapeutic agents in preclinical models. NLGP has shown a nephron-protective effect in combination with 5-fluorouracil, suggesting a potential for reduced chemotherapeutic toxicity. Its primary research indication is as an immunotherapy and antineoplastic agent for cancer, especially melanoma and other solid tumors, as well as in preneoplastic and carcinogenic processes in oral carcinogenesis.[1][2][4][5][6]
