Nefecon is a targeted-release oral formulation of budesonide, a potent synthetic corticosteroid with high first-pass hepatic metabolism. It is specifically designed to deliver budesonide to the distal ileum, targeting mucosa-associated lymphoid tissue (GALT) where excess galactose-deficient immunoglobulin A1 (Gd-IgA1) is produced in patients with primary immunoglobulin A nephropathy (IgAN). By acting locally in the gut-associated lymphoid tissue, Nefecon selectively modulates immune activity and reduces Gd-IgA1 production implicated in IgAN pathogenesis. This targeted delivery results in low systemic exposure and fewer steroid-related adverse effects compared to conventional systemic steroids. Nefecon has demonstrated clinically significant reductions in proteinuria and preservation of kidney function as shown by reduced decline in estimated glomerular filtration rate (eGFR), leading to its approval as the first disease-modifying therapy for primary IgAN at risk of rapid progression[1][4][6][7].
