Nelipepimut-s + trastuzumab is a combination immunotherapy regimen under investigation for the prevention of breast cancer recurrence in patients with HER2 low-expressing (IHC 1+/2+, FISH nonamplified) and HER2-positive breast cancer. Nelipepimut-s (NeuVax) is a peptide vaccine derived from the extracellular domain of the HER2 protein that stimulates specific CD8+ cytotoxic T lymphocytes to target and destroy HER2-expressing tumor cells. Trastuzumab (Herceptin) is a monoclonal antibody targeting the HER2 receptor, inhibiting its signaling and mediating antibody-dependent cellular cytotoxicity. The combination aims to leverage both direct anti-HER2 effects and enhanced immune-mediated tumor cell killing. Clinical trials have shown that this combination is safe with no additional cardiac toxicity compared to standard therapy alone. While overall disease-free survival was not significantly improved in all patients with low HER2 expression, significant benefit was observed in triple-negative breast cancer (TNBC) subgroups[1][2][4][5].
