Nelmastobart is a humanized monoclonal antibody targeting butyrophilin subfamily 1 member A1 (BTN1A1), a novel immune checkpoint protein primarily expressed on dormant and chemotherapy-resistant cancer cells. Unlike traditional immune checkpoint inhibitors that target PD-(L)1, nelmastobart offers a new immunotherapeutic approach for patients whose tumors are unresponsive to anti-PD-(L)1 therapy. It acts as an inhibitor of BTN1A1, potentially enabling the immune system to recognize and attack tumor cells that evade other forms of immunotherapy. Nelmastobart is being developed by STCube for multiple solid tumors, including metastatic colorectal cancer, head and neck cancer, lung cancers (non-small cell and small cell), breast cancer, ovarian cancer, bile duct cancer (cholangiocarcinoma), pancreatic cancer, adrenocortical carcinoma, anal cancer, esophageal cancer, skin cancers including melanoma and thymic carcinoma. The drug is administered intravenously and has shown acceptable safety with minimal toxicity in early-phase clinical trials[6][7][4].
