Overview

Neluxicapone (also known as ODM-104) is a reversible, peripherally-acting inhibitor of the enzyme catechol-O-methyltransferase (COMT). Developed by Orion Corporation, it was designed as an adjunctive treatment for patients with Parkinson's disease who experience motor fluctuations while taking levodopa. By inhibiting COMT, neluxicapone prevents the peripheral breakdown of levodopa into 3-O-methyldopa, thereby increasing the plasma half-life and bioavailability of levodopa. This allows for more consistent levels of levodopa to cross the blood-brain barrier, effectively extending the therapeutic 'on' time and reducing 'off' periods in patients. It has been primarily investigated in fixed-dose combination formulations alongside levodopa and a DOPA decarboxylase inhibitor like carbidopa.
