The neoantigen-loaded personalized dendritic cell vaccine is an autologous cellular immunotherapy developed by the Department of Orthopedic Oncology at Changzheng Hospital (Navy Medical University). It is designed for the treatment of hepatocellular carcinoma, particularly in patients with spinal metastases. The production process involves identifying tumor-specific mutations (neoantigens) through whole-exome sequencing and RNA sequencing of the patient's tumor tissue. Synthetic peptides corresponding to these neoantigens are then used to pulse the patient's own monocyte-derived dendritic cells *ex vivo*. Once matured and loaded, these dendritic cells are administered back to the patient, typically via subcutaneous injection, to stimulate a robust and specific anti-tumor T-cell response. This approach aims to overcome the low immunogenicity of traditional vaccines by targeting unique, non-self antigens that are not subject to central thymic tolerance.
