**Neoantigen peptides + Montanide ISA-51 VG** is a personalized cancer vaccine formulation comprised of patient-specific tumor neoantigen peptides mixed with the oil-based vaccine adjuvant Montanide ISA 51 VG. Neoantigens are tumor-specific mutant peptides designed based on the mutational profile of each patient’s tumor. These peptides are synthesized, formulated as aqueous mixtures, and then emulsified 1:1 with Montanide ISA 51 VG, a mineral oil-surfactant adjuvant. The resulting stable emulsion is administered subcutaneously. Montanide ISA 51 VG enhances both innate and adaptive anti-tumor immune responses through mechanisms that include injection site-specific inflammation, depot formation, recruitment of antigen-presenting cells, and lymphocyte entrapment. The combination boosts T cell immunity against tumor-specific antigens, aiming to induce robust and durable anti-tumor responses in cancers such as advanced non-small cell lung cancer. Development and administration proceed under GMP conditions, with the product tailored for each patient’s mutanome.
