A neoantigen-primed dendritic cell vaccine is a personalized cancer immunotherapy in which a patient's own (autologous) or donor-derived (allogeneic) dendritic cells are loaded ex vivo with peptides corresponding to tumor-specific neoantigens—peptides derived from mutated proteins unique to the patient’s tumor. These primed cells are then administered back to the patient, where they present the selected antigens to T lymphocytes in lymphoid tissues, thereby activating and expanding tumor-specific cytotoxic T-cell responses. This approach aims to induce robust and durable anti-tumor immunity by targeting truly tumor-specific epitopes that escape central immune tolerance. The therapy is under clinical investigation for various advanced solid tumors and hematologic malignancies[1][2][3][4]. Mechanistically, it leverages the potent antigen-presenting capacity of dendritic cells to stimulate both naïve and memory T-cells against cancer cells expressing these unique mutations.
