Neoantigen-reactive T cells (NRTs) are a specialized form of adoptive cellular immunotherapy that targets cancer-specific neoantigens. These neoantigens arise from tumor-specific mutations and are expressed only by cancer cells, not by normal tissues[1][2]. The fundamental principle behind NRT therapy is its ability to selectively attack tumor cells while sparing normal tissues, making it a highly precise treatment approach[1][2]. The general procedure for NRT therapy involves: 1. Identifying patient-specific neoantigens expressed by tumor cells 2. Isolating or engineering T cells that recognize these neoantigens 3. Expanding these cells in vitro 4. Reinfusing them back into the patient to target and destroy cancer cells[1] NRTs can be sourced from: - Tumor-infiltrating lymphocytes (TILs) extracted directly from the patient's tumor - Peripheral blood lymphocytes (PBLs) - T cells from healthy donors (in some protocols)[1] After selection, these cells can be either expanded and functionally modified in vitro or engineered to express neoantigen-reactive T cell receptors (TCRs)[1].
