Each component has a distinct antimicrobial mechanism: - **Neomycin** is an aminoglycoside antibiotic that binds to bacterial ribosomes and inhibits protein synthesis, leading to bactericidal activity against a broad spectrum of gram-positive and gram-negative bacteria[1]. - **Colistin** (also known as polymyxin E) is a polymyxin antibiotic that disrupts the bacterial cell membrane of susceptible gram-negative organisms by acting as a cationic detergent[2]. - **Nystatin** is an antifungal agent that binds to ergosterol in fungal cell membranes, creating pores and causing leakage of cellular contents; it is primarily effective against Candida species[2]. - **Vancomycin** is a glycopeptide antibiotic that inhibits bacterial cell wall synthesis by binding to D-Ala-D-Ala termini in peptidoglycan precursors; it has potent activity against gram-positive bacteria including resistant strains such as MRSA[8]. This combination would provide broad-spectrum antibacterial (including multidrug-resistant organisms) and antifungal coverage. Such combinations are typically used for severe or mixed infections where both fungal and resistant bacterial pathogens are suspected or confirmed.
