The combination of neomycin and rifaximin is an antibiotic therapy that has shown particular effectiveness in treating certain gastrointestinal conditions, especially those associated with methane production in the gut. Neomycin is an aminoglycoside antibiotic that works by binding to bacterial ribosomes and inhibiting protein synthesis. Rifaximin is a gastrointestinal-selective antibiotic with broad-spectrum antimicrobial activity that works by inhibiting bacterial RNA synthesis. The combination appears particularly effective against methane-producing gut bacteria, which have been associated with constipation and certain forms of irritable bowel syndrome (IBS). Clinical applications include methane-positive IBS with constipation, methane elimination, and constipation-predominant IBS (C-IBS). In clinical studies, typical dosing consisted of Neomycin 500 mg twice daily for 10 days and Rifaximin 400 mg three times daily for 10 days. The combination appears to be well-tolerated, though larger clinical trials would be beneficial to further establish its safety profile. No direct interactions between neomycin and rifaximin have been documented. Rifaximin has an excellent safety profile with minimal systemic absorption, while neomycin carries potential risks of nephrotoxicity and ototoxicity with prolonged use or in certain patient populations.
