Neomycin sulfate + bacitracin zinc + polymyxin b sulfate + hydrocortisone is a fixed-dose ophthalmic ointment combining three topical antibiotics with a corticosteroid for steroid-responsive inflammatory ocular conditions in which bacterial infection is present or there is a risk of infection. Neomycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding the 30S ribosomal subunit; polymyxin B disrupts the outer and cytoplasmic membranes of susceptible Gram-negative bacteria through interaction with bacterial membrane phospholipids; bacitracin inhibits bacterial cell wall synthesis by interfering with dephosphorylation of the lipid carrier required for peptidoglycan precursor transport; and hydrocortisone is a glucocorticoid receptor agonist that suppresses inflammatory gene expression and leukocyte-mediated inflammation. The product has been marketed as Cortisporin and is also available generically for ophthalmic use.
