Neostigmine is a small molecule cholinesterase inhibitor used primarily to treat myasthenia gravis and to reverse the effects of nondepolarizing neuromuscular blocking agents after surgery. It works by competitively inhibiting acetylcholinesterase at sites of cholinergic transmission, leading to increased levels of acetylcholine in the synaptic cleft. This accumulation enhances stimulation of both nicotinic and muscarinic receptors at the neuromuscular junction and other cholinergic sites. Neostigmine does not cross the blood-brain barrier due to its quaternary ammonium structure. It is also used for urinary retention and postoperative distention[1][4][5][7].
