Neostigmine + atropine is a pharmaceutical drug combination commonly used in anesthesia practice for the reversal of non-depolarizing neuromuscular blockade at the end of surgery. Neostigmine is a reversible cholinesterase inhibitor that increases acetylcholine levels at neuromuscular junctions, thereby antagonizing muscle relaxants and restoring muscle function[3][6]. However, increased acetylcholine also stimulates muscarinic receptors, potentially causing bradycardia and other parasympathetic effects. Atropine, an antimuscarinic agent, is co-administered to counteract these muscarinic side effects by competitively inhibiting acetylcholine at muscarinic receptors[8]. The two drugs are often administered together (but not typically as a pre-mixed product) during surgical recovery to ensure effective reversal of neuromuscular blockade while minimizing adverse cardiac or gastrointestinal effects[1][2][5].
