Overview

Nepafenac is a nonsteroidal anti-inflammatory drug (NSAID) used as an ophthalmic suspension to treat pain and inflammation associated with cataract surgery. It is a prodrug that, after topical ocular administration, rapidly penetrates the cornea and is converted by ocular tissue hydrolases to amfenac, its active metabolite. Amfenac inhibits both cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2), enzymes required for prostaglandin synthesis, thereby reducing inflammation and pain. In the European Union, nepafenac is also indicated for reducing the risk of postoperative macular edema in diabetic patients undergoing cataract surgery.
