Overview

Nepalera is a topical ophthalmic formulation of nepafenac, a non-steroidal anti-inflammatory drug (NSAID) prodrug. Marketed by SCLERA Health Care, it is primarily indicated for the treatment of pain and inflammation associated with cataract surgery. As a prodrug, nepafenac penetrates the cornea and is rapidly converted by intraocular hydrolases into amfenac, its active metabolite. Amfenac acts as a potent inhibitor of cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes, thereby blocking the synthesis of prostaglandins that mediate inflammatory responses and pain in the eye.
