Neridronic acid is a third-generation, aminohexane bisphosphonate and small molecule drug used primarily for the treatment of bone metabolic disorders. It acts by binding to hydroxyapatite crystals in bone matrix, thereby inhibiting osteoclast-mediated bone resorption. Its mechanism involves inhibition of farnesyl pyrophosphate synthase (FPPS) in the mevalonate pathway, which disrupts protein prenylation essential for osteoclast function and induces osteoclast apoptosis. This results in decreased bone turnover rate, stabilization of the bone matrix, and reduction of hypercalcemia[1][2][5]. Neridronic acid is approved in Italy for treating osteogenesis imperfecta and Paget’s disease of bone[3][4][5]. It has also been investigated for complex regional pain syndrome (CRPS), osteoporosis (including secondary forms), and other musculoskeletal disorders[4][6]. The drug has received US FDA Breakthrough Therapy designation for CRPS but is not yet approved by EMA or FDA outside Italy[5][8].
