Nerium oleander extract is a botanical extract derived from the leaves or flowers of the *Nerium oleander* plant, containing a complex profile of cardiac glycosides, most notably **oleandrin**, as well as odoroside H and neritaloside. Its primary mechanism of action is the inhibition of the **alpha-3 subunit of Na+/K+-ATPase**. This inhibition triggers downstream signaling changes that attenuate the **mTOR** pathway, including the inhibition of **Akt**, **p70S6K**, and **NF-κB**, and the suppression of **FGF-2** (basic fibroblast growth factor) export. These molecular effects result in potent anti-tumor activity, including the induction of apoptosis and inhibition of glycolysis in cancer cells, as well as antiviral and anti-inflammatory properties. Standardized extracts like **PBI-05204** have been evaluated in clinical trials for advanced solid tumors and pancreatic cancer, showing potential to bypass traditional drug resistance mechanisms such as ABC transporters.
