Overview

Nesiritide is a recombinant form of human B-type natriuretic peptide (BNP), consisting of 32 amino acids. It acts as a vasodilator and is used for the treatment of acutely decompensated congestive heart failure, particularly in patients experiencing dyspnea at rest or with minimal activity. Nesiritide binds to natriuretic peptide receptor A in the vasculature, kidney, adrenal gland, and brain, leading to increased cyclic GMP production via guanylate cyclase activation. This results in balanced arterial and venous dilation, reduced cardiac filling pressures, decreased systemic vascular resistance, increased stroke volume and cardiac output without affecting heart rate. Additional effects include lowering aldosterone, catecholamines, endothelin-1 levels and promoting natriuresis and diuresis without significant impact on serum potassium or renal function. Nesiritide was developed by Scios (now part of Johnson & Johnson) and has been approved for acute decompensated heart failure but discontinued for chronic use[1][5][6][7].
