nesiritide + furosemide

Nesiritide + furosemide is a combination of two pharmaceutical agents used primarily in the management of acute decompensated heart failure. Nesiritide is a recombinant B-type natriuretic peptide that promotes vasodilation, natriuresis, and diuresis by stimulating cyclic guanosine monophosphate (cGMP) in vascular smooth muscle cells, leading to relaxation and negative regulation of the renin-angiotensin-aldosterone system[1][6]. Furosemide is a loop diuretic that inhibits sodium-potassium-chloride cotransporters (NKCC2) in the thick ascending limb of Henle’s loop, resulting in increased excretion of water and electrolytes[4]. When used together, nesiritide can attenuate the rise in plasma aldosterone typically seen with furosemide alone while maintaining similar levels of natriuresis/diuresis as furosemide monotherapy[2][3][5]. This combination may be particularly useful for patients with heart failure who are relatively resistant to diuretics.
