Netilmicin is a semisynthetic aminoglycoside antibiotic derived from sisomicin and produced by fermentation of Micromonospora inyoensis. It is primarily used to treat serious infections caused by aerobic Gram-negative bacteria, including strains resistant to gentamicin. Netilmicin acts by irreversibly binding to the bacterial 30S ribosomal subunit, specifically interacting with four nucleotides of the 16S rRNA and a single amino acid of protein S12. This disrupts mRNA decoding, leading to misreading and inhibition of protein synthesis, which results in bactericidal activity. Compared to other aminoglycosides like gentamicin, netilmicin has similar efficacy but may have lower rates of nephrotoxicity and ototoxicity in some settings. It is not absorbed orally and must be administered parenterally or topically[1][2][4].
