Platelet-rich plasma (PRP) is an autologous biologic therapy derived from a patient's own peripheral blood, characterized by a concentration of platelets above baseline levels. In the specific clinical trial conducted by the University of Utah for acetabular labral tears, a neutrophil-poor PRP preparation is produced from 45 mL of the patient's blood and administered via a single intra-articular injection into the hip joint. The therapeutic mechanism relies on the alpha-granules within platelets, which release a variety of growth factors—including platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), and vascular endothelial growth factor (VEGF)—and cytokines that modulate inflammation and promote tissue repair and regeneration at the site of the labral injury. The neutrophil-poor formulation is specifically intended to minimize the pro-inflammatory effects associated with white blood cells, potentially enhancing the healing environment for the acetabular labrum. This treatment is being evaluated as a minimally invasive alternative for patients aged 18-49 who have not responded to conservative management.
