Overview

Next-generation neoantigen-selected tumor-infiltrating lymphocytes (TILs) is an autologous adoptive cell therapy developed by the Vall d'Hebron Institute of Oncology (VHIO). This therapeutic approach involves the isolation of T cells from a patient's tumor, followed by a selection process to identify and enrich for T cells that specifically recognize patient-specific neoantigens—mutations unique to the individual's cancer. These selected T cells are expanded ex vivo to therapeutic quantities and re-infused into the patient after a lymphodepleting conditioning regimen. The goal of this "next-generation" selection process is to improve the efficacy of TIL therapy by ensuring the infused product is highly enriched for tumor-reactive cells, potentially overcoming the limitations of unselected TIL products in epithelial and immune checkpoint blockade-resistant solid tumors.
