Nexvax2 is a peptide-based, epitope-specific immunotherapy developed as a therapeutic vaccine for celiac disease. It contains three gluten-derived peptides that target six immunodominant epitopes recognized by gluten-specific CD4+ T cells in patients who are HLA-DQ2.5 positive, which accounts for about 90% of celiac disease cases. The therapy is designed to induce immune tolerance to gluten by reprogramming pathogenic T-cells and thereby reducing or eliminating the pro-inflammatory response triggered by inadvertent gluten exposure. Nexvax2 was administered via subcutaneous or intradermal injection with dose escalation regimens shown to increase the threshold for clinical reactivity to natural exposure to gluten and ultimately restore tolerance in celiac patients[1][4][5]. Developed by ImmusanT, Nexvax2 advanced through Phase 1 and Phase 2 clinical trials but its development was discontinued after failing to demonstrate efficacy in reducing symptoms following gluten challenge[3].
