Overview

Ngb-H64Q-CCC is a recombinant, engineered variant of human neuroglobin developed as an antidote for carbon monoxide (CO) poisoning. It features the H64Q mutation to increase CO binding affinity approximately 500-fold higher than hemoglobin, along with three surface thiol substitutions (C46G, C55S, and C120S) to enhance solubility and reduce oligomerization. The drug acts by scavenging CO from carboxyhemoglobin in red blood cells and from mitochondrial cytochrome c oxidase (CcO), thereby restoring oxygen delivery and reversing the inhibitory effects of CO on mitochondrial respiration. In animal models of severe CO poisoning, intravenous administration of Ngb-H64Q-CCC rapidly reduced carboxyhemoglobin half-life, restored heart rate and blood pressure, improved tissue oxygenation and survival rates compared to controls. The molecule is being developed as a novel specific therapy for acute CO poisoning[1][2][3][5][6][7].
