Overview

NGX267 is a small molecule muscarinic acetylcholine receptor agonist that is functionally selective for the M1 subtype and also exhibits activity at the M3 receptor. It acts as a partial agonist at these receptors. The drug was developed primarily for the treatment of Alzheimer's disease and cognitive impairment associated with schizophrenia (CIAS), as well as xerostomia (dry mouth) in Sjögren's syndrome. Mechanistically, NGX267 stimulates M1 receptors in a manner similar to acetylcholine, enhancing memory and cognitive function by promoting cholinergic neurotransmission in the brain—a system relatively preserved in symptomatic Alzheimer's patients. Activation of M1 receptors by NGX267 has been linked to reductions in amyloid-beta production and tau protein phosphorylation, processes implicated in neurodegeneration. Clinical studies have shown that NGX267 can increase salivary flow (beneficial for Sjögren's syndrome) and may offer both symptomatic improvement and potential disease-modifying effects in neurodegenerative conditions[1][2][3][5][6][7][8][9].
