Overview

NHS-IL-12 is a first-in-class recombinant fusion protein (immunocytokine) designed for targeted cancer immunotherapy. It consists of two interleukin 12 (IL-12) heterodimers fused to the heavy chains of the fully human monoclonal antibody NHS76 (IgG1). The antibody component targets exposed DNA/histones in necrotic regions within solid tumors, enabling selective delivery and retention of IL-12 at tumor sites. This targeted approach increases intratumoral IL-12 concentrations while reducing systemic exposure and toxicity compared to recombinant IL-12 alone. Mechanistically, NHS-IL-12 stimulates both innate and adaptive immunity by promoting differentiation of naïve CD4+ T cells into Th1 phenotype, enhancing proliferation and cytotoxicity of CD8+ T cells and NK cells, and increasing IFN-gamma production. Preclinical studies demonstrated superior anti-tumor efficacy over recombinant IL‑12 in murine models; clinical trials have shown tolerability with evidence of immune activation but limited objective responses as monotherapy. It is being developed primarily for advanced/metastatic solid tumors[1][3][4][5][6].
