Niacin + simvastatin is a combination pharmacological regimen consisting of the lipid-lowering agent simvastatin and an extended-release formulation of niacin (nicotinic acid). This specific combination was the focus of the CARMA (Carotid Atherosclerosis Regression at Magnetic Resonance Assessment) study conducted at the University of Pennsylvania. Simvastatin is a small molecule HMG-CoA reductase inhibitor that reduces LDL cholesterol by blocking the rate-limiting step of cholesterol biosynthesis in the liver. Niacin acts through multiple pathways, including the activation of the G protein-coupled receptor GPR109A and the inhibition of diacylglycerol acyltransferase-2 (DGAT2), to increase HDL cholesterol levels and further reduce LDL and triglycerides. The combination therapy is designed to provide synergistic lipid modification to induce the regression of atherosclerotic plaques in patients with carotid artery disease.
