Nicergoline is a semisynthetic ergot derivative and small molecule pharmaceutical used primarily as a cerebral vasodilator. It acts mainly by antagonizing alpha-1A adrenergic receptor on vascular smooth muscle, leading to vasodilation and increased arterial blood flow, particularly in the brain. This results in improved oxygen and glucose utilization, reduced vascular resistance, and enhanced neural transmission. Nicergoline also inhibits platelet aggregation. It has been used for the treatment of cognitive impairment associated with cerebrovascular disorders (such as dementia), acute and chronic cerebral metabolic-vascular disorders (including cerebral arteriosclerosis, thrombosis, embolism), peripheral vascular disease (e.g., Raynaud's disease), migraines of vascular origin, retinal vascular disorders (like diabetic retinopathy), vestibular problems of vascular nature (dizziness, tinnitus), and other conditions involving impaired blood flow or cognition. Preclinical studies suggest it may improve neurogenesis via activation of the PI3K/AKT signaling pathway[1][2][3][4][5][6].
