Overview

Nicotine conjugate vaccine is an experimental immunotherapy designed to treat nicotine addiction and prevent smoking relapse by stimulating the immune system to produce polyclonal anti-nicotine antibodies[1][2][3][4][5][6]. It consists of synthetic nicotine-derived haptens chemically linked (conjugated) to immunogenic carrier proteins, and is formulated as a conjugate vaccine often including adjuvants to enhance immune response[1][4]. Upon administration, the vaccine triggers B and T cell-dependent immunity to generate antibodies that bind circulating nicotine, forming molecular complexes too large to cross the blood-brain barrier, thereby reducing the rewarding central nervous system effects of nicotine and supporting abstinence in smokers[1][2][3][4]. Multiple versions have been developed (NicVAX by Nabi Biopharmaceuticals, TA-NIC, NicQb, and Niccine), with different carrier proteins such as Pseudomonas aeruginosa exoprotein A, recombinant cholera toxin B, virus-like particles (VLPs) from bacteriophage Qβ, and tetanus toxoid[3][4][6]. Clinical trials have demonstrated safety and immunogenicity, but efficacy for smoking cessation is dependent on high antibody titers, and overall results in large trials have been modest; none are FDA-approved[2][3][4][5][6]. Developers and manufacturers include Nabi Biopharmaceuticals (NicVAX), Cytos Biotechnology/Novartis (NicQb/NIC002), and Independent Pharmaceuticals (Niccine)[3][4][6].
