NicVAX is a nicotine conjugate vaccine developed by Nabi Biopharmaceuticals for smoking cessation and the prevention of nicotine addiction. It consists of a nicotine derivative (3-aminomethylnicotine) covalently linked to a carrier protein, recombinant Pseudomonas aeruginosa exoprotein A (rEPA). The vaccine's mechanism of action involves stimulating the production of nicotine-specific antibodies. These antibodies bind to nicotine molecules in the systemic circulation, forming a large complex that is unable to cross the blood-brain barrier. By sequestering nicotine in the blood, the vaccine prevents it from reaching the nicotinic acetylcholine receptors in the brain, thereby blocking the reinforcing effects and the dopamine-mediated reward associated with tobacco use. Despite showing promise in early trials, NicVAX failed to meet its primary endpoints in two pivotal Phase 3 clinical trials, leading to the discontinuation of its development.
