Overview

Nipradilol is a synthetic small molecule non-selective adrenergic beta-blocker and nitric oxide donor, marketed primarily for ophthalmic use in Japan since 1988[1][6]. It lowers intraocular pressure (IOP) by both suppressing aqueous humor production and increasing its outflow, making it effective for the treatment of glaucoma and ocular hypertension[1][2][4][7]. Mechanistically, it counteracts both β-adrenergic and α1-adrenergic receptors, and releases nitric oxide, leading to vasodilation and possible neuroprotective effects on retinal ganglion cells via anti-apoptotic actions[1][3][4][8]. Its systemic side effects are reported as mild compared to other beta-blockers, and its known adverse reactions are mostly local (eye irritation, conjunctival congestion, superficial keratitis, itch)[1][5]. Marketed as ophthalmic solutions, it is used in a regimen of 1 drop twice daily for ocular conditions[1][2][7].
