Niraparib is an orally active small molecule inhibitor of poly (ADP-ribose) polymerase (PARP), specifically targeting PARP-1 and PARP-2 enzymes. By inhibiting these enzymes, which are involved in DNA repair, niraparib induces cytotoxicity in cancer cells—particularly those with deficiencies in homologous recombination repair mechanisms such as BRCA mutations. Niraparib is primarily indicated for the maintenance treatment of adult patients with advanced or recurrent epithelial ovarian, fallopian tube, or primary peritoneal cancer who have responded to platinum-based chemotherapy. It has also been approved in combination with abiraterone and prednisone/prednisolone for certain cases of metastatic castration-resistant prostate cancer associated with BRCA mutations.
