Overview

Nirmatrelvir + ritonavir + midazolam is a combination of three pharmaceutical agents. Nirmatrelvir is an oral antiviral that inhibits the SARS-CoV-2 main protease (Mpro), thereby preventing viral replication. Ritonavir acts as a pharmacokinetic enhancer by inhibiting CYP3A-mediated metabolism of nirmatrelvir, increasing its plasma concentration and effectiveness; it does not have direct activity against SARS-CoV-2. Midazolam is a short-acting benzodiazepine used for sedation and anesthesia; it acts as an agonist at the GABA-A receptor. The combination of nirmatrelvir and ritonavir (marketed as Paxlovid) is approved for the treatment of mild to moderate COVID-19 in patients at high risk for progression to severe disease[1][2][6]. Co-administration with midazolam requires caution due to significant drug-drug interactions mediated by CYP3A inhibition from ritonavir, which can increase plasma concentrations of midazolam and potentially lead to serious adverse effects[8].
